Vaginal Atrophy (Dryness) & UTI prophlaxis



  • Also spelled Oestradiol
  • More potent than estriol
  • Unlicensed for prophylaxis of UTI but used a lot for this 
  • FDA approved for hormone replacement therapy

Vagifem® (10mcg estradiol) vaginal tablets

  • Insert 1 daily for 2 weeks, then 1 twice weekly

Estring® Vaginal ring 

  • Insert into upper third of vagina and wear continuously; replace after 3 months; max. duration of continuous treatment 2 years
  • Releases estradiol approx. 7.5 microgram / 24 hours


  • Estriol is weaker than estradiol

Ovestin® (0.1% estriol) Cream

  • Apply 1 applicatorful daily for 3–4 weeks, then reduced to 1 applicatorful twice weekly, to be applied at bedtime.
  • Apply 1 applicatorful daily for 3–4 weeks, then (by vagina) reduced to 1 applicatorful twice weekly, to be applied at bedtime, treatment should be evaluated after 12 weeks

Ovestin® Pessaries

  • Only for vaginal atrophy 
  • 1 pessary daily for 3 weeks, then (by vagina) 1 pessary twice weekly, to be inserted at bedtime.

 Imvaggis® Pessary

  • Only for vaginal atrophy
  • 0.03mg estriol
  • Alternative to Vagifem
  • More cost effective and beneficial for patients unable to tolerate other products

Dosage Conversions

  • Ovestin® cream 1 applicatorful contains 500 micrograms estriol
  • Blissel® gel 1 applicatorful contains 50 micrograms estriol
  • Imvaggis® pessary, 1 pessary contains 30 micrograms estriol

    • For use inside the vagina, fill the applicator one-half full with the cream and apply inside the vagina, preferably before going to bed.
    • Can be used once a day or less often as needed, depending on individual symptoms. We recommend wearing a panty liner when using this product.
    • For external genital area and the vaginal opening, apply directly several times per day prn
  • Safe during pregnancy and while breastfeeding
